MIPS Extreme and Uncontrollable Exception Application Available

The Centers for Medicare and Medicaid Services (CMS) is accepting 2026 Quality Payment Program (QPP) exception applications through Dec. 31, 2026. There are two types of exception applications that allow participants to indicate why they cannot report data for one or more of the Merit-based Incentive Payment System (MIPS) performance categories:
- Individuals, groups and virtual groups can request a promoting interoperability performance category hardship exception, including when there is insufficient internet connectivity, of uncontrollable circumstances such as a disaster, practice closure, severe financial distress or vendor issues.
- A MIPS Extreme and Uncontrollable Circumstances (EUC) Exception for one or more MIPS performance categories (quality, cost, improvement activities, and Promoting Interoperability) is available for extreme and uncontrollable circumstances, defined as rare events entirely outside of your control and the control of the facility in which an eligible clinician or group practices.
