CMS Launches MAHA ELEVATE Model

The Centers for Medicare and Medicaid Services (CMS) has launched a new Medicare value-based payment model that supports evidence-based, whole-person care approaches, including functional or lifestyle medicine interventions.
CMS will fund up to 30 cooperative agreements over three years, during which time quality and cost data will be collected on whole-person functional or lifestyle medicine interventions as a support to conventional medicine. To be selected, applicants must demonstrate experience in delivering these interventions, as well as be experienced with data collection.
Additional Program Details:
- Proposals should include services not already covered by Original Medicare.
- All proposals must incorporate nutrition or physical activity as part of the design.
- Three awards will be reserved for interventions that address dementia.
- Eligible participants include, but are not limited to, private medical practices, academic organizations, and functional, lifestyle, preventive, and integrative medicine centers.
CMS will release a Notice of Funding Opportunity in early 2026 for the first cohort. The model will begin on September 1, 2026. The second cohort will begin a year later.
