Spotlight on Training - New York
NYCOM Nassau University Medical Center is the largest public teaching hospital on Long Island, New York, with approximately 650 inpatient beds. It is a Level One Trauma Center, with a Burn Unit, Hyperbaric oxygen chamber, and an officially-designated HIV specialty unit. There are approximately 350 housestaff, with medicine fellowships in endocrinology, nephrology, pulmonary/critical care, allergy/immunology, hematology/oncology, infectious disease, rheumatology, and several other specialties. The pulmonary/critical care fellowship is co-sponsored with Long Island Jewish/North Shore Medical Center. The rhematology fellowship is co-sponsored with Winthrop University Hospital. The emergency room has approximately 81,000 visits per year and serves as the medical "safety-net" for Long Island's underserved population. NUMC is a member of NYCOMEC in affiliation with NYCOM and many other New York area teaching hospitals. There is a 20+ year history of training osteopathic medical students at NUMC and remaining for their graduate medical education years. There are generally 15-20 DO students and 6-8 Stony Brook medical students on Medicine rotations at any given time. The internal medicine residency is "dual-approved" by both the ACOI and ACGME for 3 years of training, meaning that DOs that complete the program can sit for both the DO and MD internal medicine board exams. For DOs, the first year is structured as a fast-track medicine internship, meeting the requirements both for an AOA-approved internship year, and the first year of internal medicine. The residency is fully-integrated, meaning that DOs and MDs have the same patient care responsibilities and clinical learning opportunities. DOs also have the opportunity to develop their OMT skills both on site and at NYCOM. Ken Steier, DO, is the program director for the osteopathic internal medicine residency program and also an Associate Program Director in the ACGME program. Dr. Steier also serves as the Director of Medical Education at NUMC and is a full-time attending physician in the Department of Medicine. He is a recent "Teacher of the Year" as selected by both housestaff and medical students. There are many residency and fellowship options for DOs at NUMC, including physical medicine and rehabilitation, anesthesia, radiology. There are a total of 36 DOs currently in residency or fellowship at NUMC, including fellows in gastrointestinal disease, pulmonary/critical care, allergy/immunology and rheumatology. Didactic instruction includes daily morning report, daily noon conference, and weekly Medicine Grand Rounds. Individual departments have regularly scheduled lectures and conferences as well. In conclusion, NUMC is a high-volume academic medical center with a wide variety of cases and levels of severity. There is a tremendous opportunity to learn internal medicine, and become competent in all relevant procedures. As a New York hospital, resident work hours are limited by the Bell Commission rules, which are strictly enforced. The pass rate on the ABIM certification exam has been above 90% for many years. Graduates of the NUMC internal medicine residency successfully compete for fellowship positions at prestigious hospitals such as the Mayo Clinic, Johns Hopkins, and the Massachusetts General Hospital.
