Part I of the Annual Report is the Fellow's review of the program and faculty. It is confidential and is seen only by the ACOI Council on Education and Evaluation.

Part II is the Fellow's Scope of Clinical Experience, or procedure logs, in which the Fellow enters the apropriate number of procedures completed. The program director must verify these numbers before they are submitted to the ACOI.

The Program Director's Annual Report must be completed for each Fellow by the program director. The report must be reviewed with, and signed by, the Fellow prior to submission to the ACOI.

All reports must be submitted within 30 days of completion of the training year. Fellows with questions about the evaluations should speak with their program director or contact Ms. Christina Smith at the ACOI office.
